- Drivers License Number and Expiry Date
- Vehicle Registration – Number Plate
- Warrant of Fitness – Expiry Date

Driver Responsibilities include:
| I will ensure all passengers use shoulder seat belt restraints | |
| I will ensure that all passengers will get out of the vehicle on the footpath side | |
| I will ensure that children use booster seats provided by parents/caregivers or school | |
| I will ensure that children smaller than 1.48m do not sit in the front seat of a vehicle fitted with airbags | |
| I understand that it is my responsibility to comply with all road rules | |
| I will supervise students until the event leader takes responsibility for them | |
| I will not smoke, vape or use alcohol/illegal substances while driving the car or supervising students | |
| I will not use a cell phone while driving | |
| I have ensured the school has my up to date vehicle registration and licence details | |
| I will travel the route as set by the teacher in charge |
